Level 2 - Background Check

Instructions for Level II


  1. Go to: https://www.castlebranch.com/

  2. At upper right corner, click “Place Order”

  3. Enter Package# LP31

  4. Follow instructions- it will ask for your information required to complete the screen and will give you a location near you to go for the fingerprints. The cost will be $100.50.


    Tips for setting up:

     

    When you set up an account through Castle-

    There is a dashboard, you will have instructions there as first step to set up an account w DTIS-  this is where you will also need to set up an account so that you can set up where to go for fingerprinting.

    You will go through their questions- and set up an apt at a location near you and print the form to take with you of your appointment.

    Then after prints done, go back to Castlebranch to enter in date prints scanned.

    Then this will be where the final print results will be within 24-48 hours


Please note In Home Healthcare professionals are responsible to get a Level 2 background check every five years.


After you have completed your fingerprinting through Castlebranch you need to log back into dashboard and report the date prints were done.
