Discharge Reason Update

PT/OT and SLP's

When you are discharging a patient there are multiple reasons to pick from in the drop down list i the EMR. Please review below for some details:

1. We are no longer using Discharge- Other

2. Pick the best reason to discribe why- of course Discharge Met Goals and Discharge Max Potential speaks for itself.

3. Discharge- Patient Request- this could be for pt requesting dc early examples: they have too many appointments, cant commit due to family issues or spouse illness, or they are going on a long trip. When it is their choice to DC use this reason.

4. Discharge- Therapist Request- this could be for a non compliant patient, you recommend them going back to Dr, etc.

5. Discharge- out of area/ left area - is for when they move out of area or into an ALF that has their own providers

There are other options in the discharge list you can scroll through but please try to pick the one that is closest to the reason. The biggest item for 2024 is to NOT use Discharge other- and just pick either Discharge- Patient Request or Discharge-Therapist Request.
