Business Tax Receipts

As a company that is certified by Medicare, LifeCare is required to comply with all applicable federal, state and local regulations. One local regulation requirement is the use of Business Tax Receipts (formerly occupational licenses) for any individual or business that provides either goods or services within a county or city. These regulations apply to the provision of therapy services as either a business or as an Independent Contractor. The requirements vary by county and links are provided below to assist you in the application process. Please note that this is not a requirement of LifeCare but a local county regulatory requirement in which we are assisting you with compliance.
You have 30 days from time of signing your contracts to submit to us your receipt that you have applied for this BTR and paid for it. We will put receipt in your HR file until the original license is emailed or mailed to your home.
Remember this has nothing to do with LifeCare. Fill out with your personal and home information.
This BTR is a tax deduction for your business so keep receipt for your records. This BTR does renew usually every September.
