What is Fitness Plus?
Fitness Plus is a private-pay program for patients who do not meet the medical necessity criteria for continued services but simply "want" to continue with their therapist. Fitness Plus is (1) agreed to by patient and (2) allows visits to be purchased in "buckets". It is an option for every patient.
How Do I Get My Patient Started?
Speak with the patient/responsible party regarding this option and then notify Elisha of interest. She will contact patient and notify you when payment is received.
Supervising Therapist
A Fitness Plus case will be created in the EMR.
Supervising therapist will schedule Fitness Plus Visit
Be sure Waiver is on file
Load the template LifeCare Specialty Templates -> Fitness Plus Plan
Create the initial Fitness Plus Plan of Care.
Fitness Plus Visits:
Schedule the visit in the Fitness Plus Case
Visit Type = Fitness Plus
Log in/out on HUCU
What Will I Get Paid?
You get paid your standard rate for 4 units (one hour) of care.
