Effective discharge planning begins at the evaluation. It is important to recognize that preparation for discharge requires the effort of everyone on the therapy team and helps to ensure a smooth transition to an independent program.


Here are some tips and tricks to help

you prepare the patient and family for discharge.












Know # Visits in the POC

POC Visits can be found in our EMR by clicking on the "Shield Icon" in the upper left.

Countdown Visits in the POC with the Patient

Make sure that the patient is aware of the estimated number of visits in the plan of care. Especially if services are outside of the home, such as at a daycare center, communicate this with the family/caregiver. Example text:


"___ saw you for an initial evaluation and your doctor has approved ___ therapy visits. We have a lot to work on so let's get started!"


"We have 3 visits remaining before discharge. You've done great! Let's work on some home program activities"

Share After Care Options with Patient

Use the letter attached to this Post to share options with the patient and family or recommendations.

Share After Care Recommendations with Team

If patient would benefit from referral or another therapy discipline, make LifeCare Admin aware in HUCU. We can request scripts to help avoid a delay in care services.
