Resignation/Termination
The following guidelines apply to resignation and termination of service.

Resignation
As an Independent Contractor, either you or LifeCare may terminate your agreement with cause at any time, cause to be defined as misfeasance or malfeasance, and without cause upon thirty (30) days' written notice, unless both parties agree in writing to an earlier termination date.
Unless agreed to by LifeCare, any therapist who accepts and begins care with a patient must, in good faith, complete patient services until the end of the certification period or for up to 30 calendar days after written notice, whichever comes first. Failure to do so constitutes bad faith as further defined in your Agreement with LifeCare.
Termination
Therapist performance will be monitored through chart review and with individual conferences held with therapists. Remediation will be done on an individual basis when a need is identified, with documentation also maintained in the personnel record. Therapists who are found to violate Medicare regulatory requirements will be terminated and reported to the appropriate agencies as required.
Discharge/termination from service can result from:
●Violation of LifeCare Code of Ethics;
●Noncompliance with LifeCare patient care practices, policies, and procedures;
●Medicare fraud;
●Any action that conflicts with licensure and regulatory requirements.
Termination is decided on a case-by-case basis, and remedial action may be taken to improve performance when indicated, with the therapist placed on probationary status. Actions that violate legal requirements or are fraudulent will result in immediate termination with notification to the appropriate licensing board.
