Verification of Therapy Services

Verification of time with a patient is mandatory so that billing reflects actual time with a patient.

LifeCare uses HUCU, a HIPAA-compliant communication system that includes a “Check-In” and Check-Out” with a geolocator to verify the location and time spent with a patient for a therapy service. Each therapist, upon start of service, will “check in” through HUCU within the patient channel and check out at completion.


  • Services cannot be billed without an accurate match between the Treatment Verification Signature Log / HUCU and the time-in/time-out noted on the chart/schedule.

  • There are situations in which WiFi or internet connectivity is poor. In these cases, a printed Treatment Verification Signature may be used.

  • If someone other than the patient signs off on the note (such as a spouse or caregiver), have the individual sign with his/her OWN name and then document the relationship to the patient.

  • Patient signatures should never be imitated.

  • Treatment Verification Signature Logs can be uploaded directly to our EMR System. Blank Treatment Verification Signature Logs can be found on our website under Documents.
