Missed Visits, POC Compliance, & Program On Hold

Patient Missed Visits


Missed visits are to be documented in the patient record with a rationale for the missed visit included in the Appointment Notes. Missed visits can be rescheduled either by (1) adding the visit to the next week’s schedule (for example, a patient can be seen for three visits instead of 2 visits) or (2) adding to the end of the plan of care.


Is your patient cancelling for no reason or not complying with the recommended plan of care?  Discuss the need for compliance with the patient, and inform LifeCare/Supervising Therapist of your concerns. We also have a form that you can share (linked below)


Therapist Missed Visit


If you are the therapist, cancelling an appointment due to an emergency or illness, contact your patients immediately to inform them that you will not attend the scheduled appointment. Then, cancel the appointment in the EMR to document missed visits.   Write a Note in EMR to explain the reason for cancellation.  

 

Program On Hold


A rehabilitation program may be placed on hold when intervening factors prevent the patient from attending the rehabilitation services as established in the Initial Plan of Treatment. The maximum duration a program may be placed on hold is three weeks—a Program On Hold status to be documented with rationale in the client’s record. After the program on-hold period has ended, and if the patient remains a candidate for rehabilitation, readmission will be considered on a case-by-case basis. If the patient remains unable to participate in therapy, then the patient shall be discharged.
