Intake Forms & Informed Consent
The first therapist (PT/OT/ST) to see the patient is responsible for completing intake.
We're mobile, so we need our therapists' help completing the required authorizations from a patient or a patient representative. We are not able to bill for a service without these forms. The first therapist (PT/OT/ST) to see the patient must complete the intake at the time of the Initial Evaluation. We will need:
Signed Intake Package
Picture or ID of the patient (uploaded to HUCU)
Copy of insurance card(s) uploaded to HUCU
LifeCare's intake package can be completed on your phone or tablet. The patient's email is requested, and a signed copy of the form is automatically sent to the patient on completion. A printable paper version of the forms is also available.
Steps:
Get the link or QR Code for the intake package and save it to your phone/device for your use.
Open the form and review with the patient who will complete signatures on the last page.
Save the attached PDF form to upload to the EMR. How to add attachments will be demonstrated during your EMR Training.
Intakes are valid for 6 months.
