Abbreviations in Documentation

Purpose

This policy ensures that all clinical records are accurate, consistent, and easily understood by patients, staff, and external reviewers. Standardized use of abbreviations reduces the risk of misinterpretation, supports patient safety, and maintains compliance with regulatory and accreditation requirements.

Policy

Abbreviations must be clear, unambiguous, and widely recognized in healthcare practice. Therapists may use discipline-specific abbreviations and commonly accepted healthcare shorthand not listed in this policy, provided the abbreviations are commonly understood, do not pose patient safety concerns, and do not compromise medical record clarity. 

Documentation should prioritize readability and clinical accuracy over strict avoidance of abbreviations. Common clinical phrases such as "PT evaluation today with pt and wife present," "Pt tolerated treatment well," or "HEP reviewed with pt" are acceptable when their meaning is clear and would not reasonably be misunderstood by another clinician or reviewer. 

Prohibited Abbreviations

Abbreviations defined as "do not use" by JCAHO will not be used in LifeCare documentation.

Process

We review therapy documentation as part of ongoing URQA and compliance efforts.   We will ask for corrections to meet regulatory criteria if the standard is not met.
