CheckList: Progress Reports

Key Points


Medicare Regulations require a Progress/Supervisory Visit every 10 treatment visits which allows for 8 therapy only visits between supervisory appointments. Please schedule your patients at a minimum:

  • Once every 30 days for "rehab" patients scheduled 2-3 times/week

  • Once every 30 days for "maintenance" patients who are scheduled 1 time/week.

A Supervisory Visit does not need to replace an assistant's therapy visit, but can be an "additional visit" that week.


Steps


Schedule appointment with patient and add to your EMR calendar so team members are aware of the appointment;


Choose the correct visit type

  • Progress/Supervisory Visit

  • Progress With Re-Certification

Load Chart Template

  • Arrive the Visit and go to the "Paper" Icon in upper right to access the Templates and load the correct template for your visit (Progress or Progress with Re-Certification)

Complete the Note

  • Update treatment goals.

  • Add procedures

  • Sign the note

Add Case Details

  • Click on the Case Name to take you to the Case Details

  • Add Progress Report (to reset visit count)

  • Add POC (to reset certification period)


When To Complete A Re-Certification:


Medicare guidelines state that a Re-Certification of the Plan of Care shall be done when:

  • The initial certification period is going to expire and continued skilled therapy is medically necessary.

  • When additional visits are needed to achieve the therapy goals

  • When there is a change in the patient’s condition that requires a revision of the patient’s therapy goals (ie a new condition). .

The maximum length of time for a re-certification period is 90 calendar days for Medicare. United HealthCare re-certifications are only 60 days. LifeCare does recommend that you use the entire certification period for plans of care. This will help prevent you from needing a re-certification for a date only due to unexpected delay (such as from holidays or illness).
