Alternate Evaluation Templates

Overview
Each therapy discipline has a "default" chart template which a general overview of a patient's functional mobility (PT) and ADLs (OT). If you are treating a specific condition or anatomical area, alternate evaluation templates can be used.
Alternate templates can be selected from the Chart Templates under the category name "LifeCare Alternate Evaluation Templates". Each evaluation template is labelled with both area and the functional tool that is included (i.e., Shoulder with SPADI).
To Use The Alternate Evaluation Templates:
Schedule and arrive the patient for the visit.
Under the Templates, first add the Evaluation Frame Template (PT or OT)
Then add a second template for the evaluation that you wish to use (i.e., Hip, Knee, Foot & Ankle, Shoulder, Elbow, Hand, Vestibular, etc).
